Other Information:Samuel enlisted on 22nd February 1915. He was 20 years and 4 months old; was a Shipwright by trade; and was living in 20 Galmpton, West Brixham, Devon at the time. He was posted to serve with the 90th Field Ambulance but transferred to the 141st Field Ambulance, embarking at Southampton on 10th August 1915, then disembarking at Rouen the next day, he entered the war in France on 11th August 1915. On 20th November 1916 he was admitted into the 1/1st South Midland Casualty Clearing Station suffering with appendicitis. The next day, on 1st December 1916, he was transferred via No 5 Ambulance Train to No 12 General Hospital in Rouen. He remained there until 11th December, when he boarded the hospital ship "St George" and returned to the UK. On 22nd August 1919 he was discharged "no longer physically fit for war service".
